Siouxland Community Health Center provides primary health care services to individuals and seeks to address the major health problems of the community. Physicians, physician assistants, nurse practitioners, dentists and other health professionals strive to prevent disease, promote health, and treat patients who are ill or injured. Working with other community groups, the Health Center develops and designs programs to improve community health based on individual needs. |

Programs Child Health & WIC—A program which provides health screenings for children from birth to 21 years. Coordinates with Women, Infant and Children(WIC) nutrition services.
Vaccines For Children (VFC)—A service providing vaccines at no cost to children birth to 18 years who are not covered by insurance.
HIV/AIDS—A program that provides primary health care to individuals living with HIV/AIDS as well as offering early intervention through HIV testing and outreach. Click here for more details.
DHS—An Income Maintenance Worker for Iowa is now located at the Health Center to assist with Title 19 and HAWK-i applications.
Interpretive Services Services for the hearing impaired are also available when needed.
Pharmacy The pharmacist works closely with providers, staff and patients in providing education and consultative services.

Dental Service Please take the time to find out about our
I-Smiles program for children 12 years of age and younger! Emergency services are also provided on a walk-in basis at time permits. Referral services are available for difficult cases and situations that are beyond the scope of practice for a general dentist.
In Case of Emergency If an emergency occurs after office hours, you may call the operator at St. Luke's Regional Medical Center, 712-279-3500. The provider will return your call quickly. If you need to be seen, the provider will direct you to go to the Emergency Room.
A phone call to the provider prior to going to the Emergency Room will help you to be seen more quickly at the hospital.

Staff
Social Services Outreach activities promote HAWK-i enrollment and patient education/prevention programs.
Financial Services The Health Center files insurance claims of your behalf. You are to bring your insurance card, Title 19 and/or Medicare card with you to each appointment.
A sliding fee scale is available for patients with no insurance or Title 19. Patients interested in applying for this discount need to bring financial information with them to their first appointment.
