Kelly Bean, ARNP
Kelly Bean started with the Siouxland Community Health Center as an Advanced Registered Nurse Practitioner in September 2018 at the South Sioux City location.
Bean graduated from the Creighton University in 2006 with a Master of Science in Nursing degree and Clinical Nurse Specialist in Community Health.
Bean has practiced in the areas of Family Medicine, Emergency Room Medicine, Acute Care and Geriatrics.
Jill Bundy, PA-C, MPAS
Jill Bundy joined the Siouxland Community Health Center as a Physician Assistant in March of 2016.
She earned her undergraduate degree in Biology from the University of Nebraska in Lincoln in 2008 and completed her Master of Physician Assistant Studies degree in 2012 at the University of Oklahoma in Tulsa.
Molly Earleywine, PA-C
Molly Earleywine has been a physician assistant at the Siouxland Community Health Center since May 14, 2001.
Earleywine completed her education at the University of South Dakota-Vermillion. She received a Bachelor of Science degree in 1989 and graduated Summa-Cum-Laude from the PA program at the University of South Dakota-School of Medicine in August 2000.
She is a member of the American Association of PA’s and the Iowa PA Society.
Staci Ervin, ARNP
Staci Ervin has been a Family Nurse Practitioner at Siouxland Community Health Center since 2013.
Staci has been a registered nurse since 2004 and earned her Bachelor of Science in Nursing in 2009 from Briar Cliff University, where she graduated Cum Laude. In 2012, Staci graduated at the top of her class with her Master’s of Science in Nursing with a specialty in Family Practice and was the recipient of the Academic Excellence Award. She is certified with the American Academy of Nurse Practitioners.
Staci has a special interest in women’s health, pediatrics, and chronic disease management
David Faldmo, PA-C, MPAS
David N. Faldmo, PA-C, MPAS, is the Quality Director/Medical Director of Siouxland Community Health Center. Faldmo, who has been at SCHC since 1993, received his medical training at the Roy J. and Lucille A. Carver College of Medicine, University of Iowa.
Faldmo completed a Community Health Center Executive Fellowship through Kansas University Medical Center in 2011.
He is a member of the American Association of Physician Assistants and the Iowa Physician Assistant Society.
Tonya Flaugh, ARNP, FNP-C
Tonya Flaugh was a Family Nurse Practitioner at Siouxland Community Health Center from 2014-2017. She provided care in Cardiology before coming back to SCHC in 2019.
Tonya has been a registered nurse since 2005 and earned her Bachelor of Science in Nursing in 2009 from South Dakota State University. In 2013, Tonya received her Master’s of Science degree in Nursing with a specialty in Family Practice from Briar Cliff University. She is certified with the American Academy of Nurse Practitioners.
Tonya has a special interest in Cardiology and is certified to treat opioid use disorders through our Medication Assisted Therapy.
Dylan Greene, M.D.
Dr. Greene joined the clinical staff at the Siouxland Community Health Center in the summer of 2016.
The Idaho native is a graduate of the Siouxland Medical Education Foundation in Sioux City. He earned his bachelor’s degree in biology from Creighton University, where he graduated Magna Cum Laude. He went on to obtain his medical degree from Creighton University School of Medicine in 2013.
Dr. Greene is certified in Advanced Cardiac Life Support, Pediatric Advanced Life Support, Neonatal Resuscitation, Advanced Life Trauma Support and Advanced Life Support in Obstetrics.
Randy Guerdet, PA-C
Randy Guerdet has been a Physician Assistant at the Siouxland Community Health Center since June 2002. He received his Bachelor of Science and Master of Divinity Degrees from St. John’s University. He graduated from the University of Iowa College of Medicine as a Physician Assistant.
Mr. Guerdet worked at a rural health clinic and Family Practice Residency Program prior to coming to SCHC. In addition to seeing patients in Family Practice, he is certified in HIV medicine with the American Academy of HIV Medicine.
He is bilingual in Spanish.
Gary Hattan, M.D.
Gary Hattan joined Siouxland Community Health Center in 1998 after having worked there for several months during residency training. He graduated with a Bachelor of Science degree in Biology from California State University at San Bernardino in 1989. After graduation, he worked for a year in immunology research at City of Hope in Duarte, CA and surgery research at Loma Linda Medical Center in Loma Linda, CA. He graduated from St. George’s University School of Medicine finishing his clinical studies in New York City and Newark, NJ. He completed residency at Siouxland Medical Education Foundation in 1998.
Dr. Hattan is Board Certified in Family Medicine and is a member of the American Medical Association, American Academy of Family Physicians, Iowa Academy of Family Physicians, and Woodbury County Medical Society.
Chris Jacobs, ARNP, FNP-BC
Jacobs started with the Siouxland Community Health Center as an Advanced Registered Nurse Practitioner on Aug. 8, 2010. He received his medical training at Briar Cliff University in Sioux City, and is a member of Sigma Theta Tau.
Cyndy Johnson, ARNP, PMHNP-BC
Cyndy graduated from the University of Alabama 2016, and also graduated with an Associates Degree from Western Iowa Tech. She has 30 plus years of experience working with mental health and substance abuse patients in the Siouxland area. Cyndy is married with two grown children, and has two grandchildren.
She was raised in Lamoni, Iowa, but moved to Sioux City in 1981. Previously, she worked at MercyOne for 21 years as a behavioral health RN. From 2005 to 2008, she taught nursing at Western Iowa Tech. Cyndy primarily treats adult patients, but accepts patients ages 14 and above.
Lori Krause, PA-C, MPAS
Lori Krause, a physician’s assistant, has been at Siouxland Community Health Center since 2006. She received her PA-C certificate at the University of North Dakota in 2000, and her Master of Physicians Assistant degree at the University of Nebraska Medical Center in 2004. She also passed the PA certification for surgery in 1999, and is BLS certified. Lori is licensed in Nebraska and Iowa.
Krause earned national certification in Otolaryngology-Head and Neck (CORLN) in 1994. Before she became a PA, Lori was a nurse in the ICU, CCU, ER, Surgery, and ENT units. She graduated from Nebraska Methodist Hospital School of Nursing in 1985, and graduated Magna cum Laude from Briar Cliff University with a BSN in 1999. She has also taught at WITCC in their ADN and CNA programs.
Lori has also worked at the VA Clinic, and Family Healthcare of Siouxland. She is a member of Sigma Theta Tau International, Phi Theta Kappa, American Association of PA’s, and the Iowa PA Society.
Mary McGowan, ARNP
Mary McGowan graduated from the University of Iowa in 1994 with a Bachelor of Science in Nursing degree. She continued her education at the University of Nebraska Medical Center where she earned a Master of Science in Nursing degree in 1999.
McGowan joined the Siouxland Community Health Center in 2015. She has practiced in the areas of Family Medicine, Emergency Room Medicine, Acute Care and Pulmonology.
Tom Morgan, M.D.
Dr. Thomas Morgan was born and raised in Detroit, Michigan. After two years at Michigan State University, he transferred to Northern Arizona University and graduated with a B.S. in Medical Microbiology in 1983. He began graduate school at NAU but chose to marry, Sarah, and enter the Peace Corps. They served in Sierra Leone, West Africa from 1986-1987 as fresh water fish aquaculturists. Upon returning, he completed a MS in Agribusiness from Arizona State University. They moved to Iowa in 1991, and he earned a Physician Assistant degree from the University of Iowa in 2000.
Upon graduation, he practiced on the Omaha Indian Reservation for five years. In 2010, he graduated from Winsor University in St. Kitts with his MD. Dr. Morgan was trained in Family Medicine at the Siouxland Medical Education Foundation residency program, and graduated in 2013. He worked for the residency for 4 years, and then worked for the Omaha Tribe of Nebraska in Macy, NE for 2 years. He joined Siouxland Community Health Center in 2020.
Dr. Thomas Morgan is Board Certified in Family Medicine, ACLS, ATLS, PALS and ALSO.
Dena Neiman, ARNP
Dena Neiman joined the staff of the Siouxland Community Health Center as an Advanced Registered Nurse Practitioner (A.R.N.P).
Ms. Neiman is a resident of Lawton, and has worked in the Siouxland area in a number of capacities. Prior to her graduation from Clarkson College, she worked for nine years as a registered nurse at Siouxland Urology. After earning her Master’s Family Nurse Practitioner degree at Clarkson College, Neiman worked as an A.R.N.P. at Family Healthcare of Siouxland, and most recently Winnebago Indian Health Services.
Ms. Neiman is a member of Sigma Theta Tau National Honor Society.
Amy Payne, PA-C, MPAS
Amy Payne has been a Physician Assistant at Siouxland Community Health Center since April 1, 2020.
Amy completed her bachelor of science degree from Morningside College in 1991. After graduation, she worked in a Social Services capacity for over 10 years. She then attended the University of South Dakota PA program, where she earned her Master’s in Physician Assistance Sciences.
Upon her graduation from USD, she worked at Indian Health Services Winnebago Hospital for 5 years where she was awarded PA of the year for all of IHS. Following her employment at IHS, she worked at Family Health Care of Siouxland as a PA and was their Diabetic Educator for 9 years.
She is currently a member of the AAPA and the SD PA society.
Ann Pick, ARNP
Ann Pick has been a Family Nurse Practitioner since May 1, 1994. She received her master’s degree in nursing and completed the Nurse Practitioner program at South Dakota State University-Brookings.
Pick was named Nurse Practitioner of the Year in 2000 by the National Health Services Corp. In addition, she was a NW Iowa representative for the Iowa Nurse Practitioner Society (INSPS), of which she was a founding member.
Michael Piplani, Chief Medical Officer, M.D.
Dr. Michael Piplani started at SCHC in 2014 as the Chief Medical Officer. In addition to his administrative role as CMO, he sees patients at SCHC. He is board certified by the American Board of Family Medicine and is active in all areas of care with the exception of obstetrics. He also practices acupuncture. He is a member of the American Academy of Medical Acupuncture.
Dr. Piplani received his undergraduate degree from Cornell University and graduated from Albany Medical College as a Doctor of Medicine with Distinction in Research. He completed his residency at Albany Medical Center in the Department of Family Practice. He is a member of the American Academy of Family Physicians and the Iowa Academy of Family Physicians.
Elizabeth Pratt, ARNP
Elizabeth graduated from the University of Nebraska Medical Center in 2002 with her MSN and Family Nurse Practitioner degrees. She has worked in family practice and occupational health care ever since she graduated.
Joni Roberts, MSN, CNM
Joni Roberts joined the Siouxland Community Health Center in May 2019. She received her BSN at the University of Iowa (2004); practiced as a nurse in labor and delivery at Sanford, Sioux Falls, SD before returning to school, then graduated with MSN with specialty of midwifery – Emory University (2008)
Joni is a member of the American College of Nurse Midwifes.
Kelly Ryder, M.D.
Dr. Ryder joined the clinical staff at the Siouxland Community Health Center in the summer of 2017.
She is a graduate of the Siouxland Medical Education Foundation in Sioux City. She earned his bachelor’s degree in Mathematics from Creighton University. She went on to obtain her medical degree from Creighton University School of Medicine in 2014.
Dr. Ryder is certified in Advanced Cardiac Life Support, Pediatric Advanced Life Support, Neonatal Resuscitation, Advanced Life Trauma Support and Advanced Life Support in Obstetrics.
Daga Said, Pharm.D, MS, CDE, Clinical Pharmacist
Dr. Daga Said is the Ambulatory Care Clinical Pharmacist at Siouxland Community Health Center and a Clinical Assistant Professor at University of Nebraska Medical Center in the department of Pharmacy Practice. He received his Doctor of Pharmacy degree from Washington State University and Master of Science in Medication Therapy Management with a concentration in Chronic Disease Management from University of Florida. He is also a Certified Diabetes Educator. At his clinical practice, Dr.Said provides care for patients in chronic disease state management (including diabetes, hypertension, hyperlipidemia, smoking cessation, and anticoagulation) under Collaborative Practice Agreement protocol. Dr.Said is also involved in implementation and enhancement of clinical activities including Insulin-Pump Therapy Management, Interpretation of Continuous Glucose Monitoring (CGM), drug therapy consultation to the providers, and serving as SCHC Diabetes Education Program Coordinator. Dr. Said provides didactic lecture of Pharmacotherapy course for third year professional Pharmacy student at University of Nebraska Medical Center College of Pharmacy with an emphasis on diabetes, Insulin Pump Therapy and Continuous Glucose Monitoring and cardiovascular risk reduction and he precepts fourth year pharmacy students at Siouxland Community Health Center for Ambulatory Care Advanced Practice rotation. His area of interest includes diabetes and technology, cardiovascular risk reduction and integration of Comprehensive Medication Management in Patient-Centered Medical Home. Dr.Said has been active member of American College of Clinical Pharmacy (ACCP), American Society of Consultants Pharmacist (ASCP), American Association of Diabetes Educators (AADE), and American Diabetes Association (ADA) for several years. He has been serving on various ACCP Practitioner and Research Network Committee including Ambulatory Care, Endocrine and Metabolism and Cardiology for more than three years. Dr.Said is fluent in 3 languages, English, Afan Oromo, and Amharic.
Rigpa Shunya, ARNP
I specialize in treating wide spectrum of psychiatric disorders. Especially Generalized Anxiety, Panic disorder, PTSD, Major Depression, ADHD, Bipolar mood disorder, Anger problem, Marital relationship issues, how to practice Forgiveness. I help provide methods to transcend harmful negative destructive emotions and heal oneself and others.
My treatment method is evidenced based, working in collaboration with my provider team and applying psychotropic medications and Therapy such as Mindfulness Meditation and higher practice called Super Consciousness.
My approach is client focused. Client being the director of their treatment. I see every human being as having Enlightenment as their true nature, thus capable of expressing Enlighten qualities, such as Boundless Compassion, Kindness, patience, Love for all. The obstacle to our Enlighten State that obscure its Brilliant light and qualities is the first layer of mind that is programmed in us that I call ‘Ego Personality Layer of Mind.”

LaVone Sopher, MSN, ARNP, FNP-BC
LaVone graduated with her BSN from Morningside College in 1979. She received her MSN from the University of Texas at Arlington in 1982. About twelve years later, she graduated from South Dakota State-Brookings with her post master’s FNP degree. In 1994, she received her FNP certification through AANC. LaVone is certified to perform DOT exams, and is a member of AANP, ANA, and INA.
Dr. Heather Stehr, ARNP, FNP-BC
Dr. Heather Stehr joined the Siouxland Community Health Center as an Advanced Registered Nurse Practitioner in November of 2018.
Born and raised in Northwest Iowa, Dr. Stehr completed her Masters of Science in Nursing Education in 2014, Post-Master’s Certificate as Family Nurse Practitioner in May 2018, and her Doctorate of Nursing Practice in May 2020, all from Briar Cliff University in Sioux City, Iowa. She is a member of AANP and ANCC, and Sigma Theta Tau.
Certified through the American Nurses Credentialing Center as a Family Nurse Practitioner, Dr. Stehr welcomes patients of all ages. Additionally, Dr. Stehr serves as an assistant professor at Briar Cliff University in the graduate nursing department. In March 2020, Dr. Stehr was named Provider of the Year at Siouxland Community Health Center. Upon completion of her Doctorate of Nursing Practice, she was awarded the 2020 Award of Excellence in Recognition of Her Outstanding Practice Innovation Project, which was completed at the Siouxland Community Health Center. Dr. Stehr is a certified Medication Assisted Treatmeant (MAT) provider and practices MAT at the Siouxland Community Health Center.
Shannon Stewart, ARNP
Shannon joined the Siouxland Community Health Center as an Advanced Register Nurse Practitioner in January of 2016. She received her post masters certificate in Family Nurse Practitioners from Briar Cliff University in 2012.
She is accredited through the American Nurses Credentialing Center. In 2008, Shannon earned her Masters in Nursing Education from the University of Phoenix. Prior to obtaining her Master’s degree and post masters certificate, she had been a registered nurse for 20 years. She is currently working on her DNP in Clinical Practice Tract through Morningside College to be completed in 2021.
She has experience in infectious disease services, neurology, and neurosurgery, and is a certified HIV Specialist provider through the AAHIVS.
Shannon is a member of the Iowa Nurse Practitioners Society and AAHIV.
Rebecca Walding, ARNP
Rebecca joined us in April of 2017. She specializes in child and family mental health and has been practicing since 2009. Graduating with her FNP in 2007 from Briar Cliff, she went on to graduate from Arizona State University in 2009 as a Child/Family Psychiatric Nurse Practitioner. She is a member of Sigma Theta Tau International Society of Nursing and Iowa Nurse Practitioner Association. Rebecca is proud to provide top-notch mental health services to Siouxland and enjoys staying active with her family.
Kristi Walz, M.D., AAHIVS, FAAFP
Dr. Walz has been a physician at the Siouxland Community Health Center since August 2002. She received her undergraduate degree at the University of South Dakota (USD) in 1995, and went on to attend the USD School of Medicine, where she graduated with a medical degree in 1999. She completed her residency at the Siouxland Medical Education Foundation in 2002.
Walz has had her HIV Specialist certification since 2004, was medical director from 2007 to 2012, and has been on various committees at UnityPoint Health-St. Luke’s and Mercy Medical Center.
Dr. Walz is board certified in Family Medicine, is a member of American Academy of Family Physicians, Iowa Academy of Family Physicians, and the President of Woodbury Medical Society.
Tyler J. Wendel, D.O.
Dr. Tyler J. Wendel is a Northwest Iowa native having graduated from Schaller-Crestland High School in Early, Iowa. He earned a B.S degree from the University of Iowa in 2009 and his D.O. from Des Moines University in Des Moines, Iowa.
Dr. Wendel then came to the Siouxland Medical Education Foundation where he completed his residency. He is certified in Advanced Cardiac Life Support, Advanced Life Support in Obstetrics, Advanced Trauma Life Support, Neonatal Resuscitation Program, Pediatric Advanced Life Support and Electronic Fetal Monitoring.
Elizabeth Wessling, PA-C, MPAS
Elizabeth Wessling has been a physician assistant in the Full-Time Walk-In Clinic at Siouxland Community Health Center since Sept. 4, 2012.
Elizabeth attended Mount Marty College and earned her Bachelor of Arts degree in Biology in 2005 and received her Master’s Degree in Physician Assistant Studies from the University of South Dakota in 2007. She also has specialty training in Pain Management from the University of California San Francisco.
Cliff Wilkinson, PA-C
Cliff has practiced at the Siouxland Community Health Center since 2008. He has an extensive background with neurosurgery and orthopedics and is well-versed in common problems such as neck and back pain, arthritis and tendinitis.
Cliff earned his undergraduate degree in Electric Engineering Technology from Brigham Young University and his PA-C from the University of South Dakota in 1997 and is a member of the American Academy of Physicians Assistants.
Jamie Wright, ARNP
Jamie Wright joined the Siouxland Community Health Center as an advanced registered nurse practitioner in November 2017. She is a 2011 graduate from Morningside College in Sioux City, Iowa where she earned her Bachelor of Science in Nursing degree and a 2017 graduate of her alma mater with a Master of Science in Nursing degree with a specialty in family practice. She is board certified in family practice through the American Academy of Nurse Practitioners
Brenda Hausman-Miller, D.D.S., Dental Director
Dr. Brenda Hausman-Miller has been a full-time dentist at the Siouxland Community Health Center since July of 2006.
Hausman-Miller obtained her bachelor’s degree in biology through Creighton University, her doctorate of dental science through the University of Iowa and her advanced education in general dentistry through the Naval Dental Center.
She came to the SCHC with four years of practical experience as a dental officer with the U.S. government.
Ashley Huber, D.D.S.
Ashley Huber, D.D.S., has recently joined the team of dentists at Siouxland Community Health Center.
The Dakota Valley High School graduate earned her bachelor’s degree from Minnesota State University and her doctorate of dental science from the University of Nebraska College of Dentistry.
Letitia McCarthy-Clifford, D.D.S.
Dr. McCarthy came to Siouxland Community Health Center in June of 2017. She received her B.S. and D.D.S. degrees from Creighton University. She moved here from New Mexico where she practiced at a Federally Qualified Health Center for 25 years. She made the move to be closer to her children who live in Omaha and are we also Creighton Graduates.
Dave Motz, D.D.S.
Dr. Motz returns to SCHC after working here from 2001-2003. At that time, the current building was a parking lot. Things have changed a bit since then. Dr. Motz is a graduate of Iowa Wesleyan College and the University of Iowa Dental College. He brings 20 years of experience in both public health and private practice dentistry to the dental clinic.
Alison Uhl, D.D.S.
Dr. Uhl has been a dentist at the Siouxland Community Health Center since July 2011. She received both her bachelor’s degree in chemistry and her D.D.S. degree from the University of Iowa. Following her graduation from dental school, she earned a certificate through the dental general practice residency program from the University of Iowa Hospitals and Clinics.
