Chris Jacobs started with the Siouxland Community Health Center as an Advanced Registered Nurse Practitioner on Aug. 8, 2010. He received his Family Nurse Practitioner training at Briar Cliff University in Sioux City and is a member of Sigma Theta Tau. He is board certified for Advanced Diabetes Management and is an HIV Specialist. He is also a Medical Director.
