David N. Faldmo, PA-C, MPAS, is the Chief Population Health Officer and the Physician Assistant Medical Director of Siouxland Community Health Center. Faldmo, who has been at SCHC since 1993, received his medical training at the Roy J. and Lucille A. Carver College of Medicine, University of Iowa.
Faldmo completed a Community Health Center Executive Fellowship through Kansas University Medical Center in 2011.
He is a member of the American Association of Physician Assistants and the Iowa Physician Assistant Society.
