Sign Up for MyChart:
Signing up for an account is easy and, in most cases, only takes a few minutes!
(If you signed up for the patient portal in the old system, you will need to set up a new account with MyChart.)
- Click the “Sign Up For MyChart” button above or download the MyChart mobile app to your phone or tablet.
- If you do not have an activation code, choose the “Sign Up Online” option.
- When using the MyChart mobile app, search for Community Health Care, Inc. and add CHC as your organization.
Billing:
Billing statements for all SCHC services can be paid using the MyChart patient portal by clicking HERE
